Your First Visit
Your first appointment will consist of a consultation with the doctor explaining your diagnosis and treatment options. You will receive a complete "dental physical" so that we can gather a detailed diagnostic record of your mouth and teeth.
Your examination will include:
- Medical and Dental History
- Blood Pressure Screening
- An Oral Cancer Screening
- Digital Photographs of Your Face, Profile, and Teeth
- Evaluation of Your Existing Restorations
- Functional Diagnosis of the TMJ (your jaw joint)
- Gum-Depth Measurements
- Customized Treatment Planning
- Patient Education on Findings
We need patients to bring referral slips, a list of medications, and their dental insurance cards. All patients under 18 must have a parent or legal guardian present. Please alert the dental office staff if you have an existing medical condition, including artificial valves or joints, rheumatic fever, or are pregnant. If your dentist or physician has taken recent X-rays, please have them forwarded to our Hackettstown dental office, or you may pick up the films and bring them with you. Should we need more information, additional films can be taken at our facility.
During your second appointment, Dr. Kakkar will review his findings and explain his treatment recommendations. You’ll have time to ask questions, so you can understand any issues we have discovered and our proposed treatment.
Dental Patient Forms
To save time at your first appointment, we encourage you to take a few moments now to fill out your patient forms. If you do not wish to complete your forms before your visit, make sure to allow a little extra time before your scheduled appointment to get these completed.
We will be happy to assist you in processing and filing dental insurance claim forms for most major providers. Please let us know if you wish to use dental insurance and we’ll help you get the process started before your appointment. We will accept most insurance, but we offer in-network coverage for all of the following plans:

We accept cash, checks, and credit cards (Visa, MasterCard, Discover, American Express), and our dental office participates with CareCredit, a third-party financier that offers low and no-interest loans for qualified patients. Our financing coordinators may also offer flexible payment plans on a case-by-case basis.

Don’t have access to a traditional dental insurance plan? No problem. We’ve got you covered. Ask about our in-house savings plan. Each patient pays $350 per year, $300 for senior citizens, and they receive the following coverage:
